Identification

The state of Arizona requires valid Arizona identification. You may bring along your ID or email a copy of any of the following identification forms:

  • Valid Arizona Identification
  • Arizona Driver’s License
  • US passport

Medical Records

Please bring your recent medical records- Chart Notes or Progress Notes from the past 12 months. The state requires our physicians to review your recent records in order to properly confirm the necessity of your medical marijuana certificate.  

If you do not have recent medical records, Affordable Evaluations can assist you with a Pain Evaluation. The evaluation costs $45 and can be administered on the same day as your application. For more information or to set an appointment, contact us 480.420.4756.

Payment

  • Cash or credit/debit card for AEAZ Fees
  • Credit/debit card for State Fees

The Affordable Evaluations office prefers fees to be paid in cash. We can accept Visa, MasterCard, Discover and American Express for a convenience fee of $2.50. The State Department fees must be paid on a Visa or MasterCard credit/debit card.
